A police watchdog has launched an investigation into a former chief constable and three other officers following the controversy over the Maccabi Tel Aviv supporter ban.

Fans of the Israeli club were barred from travelling to November's Europa League match against Aston Villa at Villa Park because of safety concerns stemming from an inaccurate West Midlands Police report.

The report contained eight errors, including overstating Maccabi supporters' role in 2024 disorder in Amsterdam and mentioning a non-existent match against West Ham, reportedly produced by Microsoft Copilot.

Former West Midlands Police chief Craig Guildford is being investigated for gross misconduct, the Independent Office for Police Conduct said, over claims he failed to ensure due diligence regarding the document.

Another officer and a staff member also face potential gross misconduct investigations over the mistakes, alongside two officers involved in match planning who face potential misconduct allegations.
